Experience Care Provider CNA since 2005 Has extensive experience and skills working with dementia clients as she had worked previously in a specialized setting for memory care clients. Also worked in Assisted Living community. Will accept residents at care home level and allow them to age in place including hospice/end of life care so that the resident will not have to transfer out when requiring hospice care. Staffing (day & night): 2 or more staff at home to provide 24/7 care and supervision

About This Home

This Adult Residential Care Home is in Village Park (Waipahu) neighborhood. Eateries and shopping center within just a few minutes from home. Only minutes to QMC-West and Pali Momi Medical Center. Rooms available: large private room and a shared female room (2 people in a room). Highlights: -The large private room has two large windows providing lots of brightness in the room and it comes with a view of the Kunia Golf course with lots of greenery. -Private room also has a large futon sofa in the room and a closet -Care provider has two small very friendly dogs that are very affectionate and joyful -Care Provider is in process of adding a covered lanai area for residents to enjoy sitting outdoors for fresh air. *Fenced around perimeter of property. *May add portable A/C upon request. SERVICES PROVIDED:

24 hour supervision
Meals: nutritious home cooked meals 3 times a day and snacks. Per client's preferences & dietary restrictions.
Activities of Daily living: assistance with feeding, walking, transfers, bathing, toileting/incontinence care
Medication Management
Monitoring Vital Signs
Exercise: Offered daily and as needed; per doctor’s instructions
Activities: Individualized per client's preferences
Laundry
Transportation: Caregiver may provide free transportation as part of their service to doctor’s appointments for client’s able to transfer in and out of car. For clients requiring more assistance, caregiver can arrange for private transport which generally is an out of pocket expense for families/clients.
