Has taken care of clients with various medical conditions, and has background working in long term care and hospice/end of life care. Care Provider also has secondary caregivers to provide additional assistance with care.

About This Home

This Expanded Care Home is conveniently located in Salt Lake. Only minutes from town and major hospitals. This home is easily accessible from town and Leeward side. This home has a very large living room where clients can walk and also a relaxing lanai overlooking a beautiful tropical stream where residents can contently watch koi fish and ducks in the distance. This private room is bright, and comfortable and faces out to the back of the home where the stream is within view. Room size: 100 sq. ft. SERVICES PROVIDED:

24 hour supervision
Meals: nutritious home cooked meals 3 times a day and snacks. Per client's preferences & dietary restrictions.
Activities of Daily living: assistance with feeding, walking, transfers, bathing, toileting/incontinence care
Medication Management
Monitoring Vital Signs
Exercise: Offered daily and as needed; per doctor’s instructions
Stimulating Activities and Crafting: Individualized per client's preferences
Laundry
Transportation: Caregiver may provide free transportation as part of their service to doctor’s appointments for client’s able to transfer in and out of car. For clients requiring more assistance, caregiver can arrange for private transport which generally is an out of pocket expense for families/clients.
