Halawa Care Home LLC Care Provider: Marites Collado Maximum: 5 residents (2 expanded level, and 3 arch level)   This Expanded Adult Residential Care Home is located in Halawa. Conveniently located 15 minutes to town and to Windward side(Kaneohe/Kailua). Highlights of Home: -Care Provider lives on property and works full time at home and her spouse is also full time at home assisting the residents with care -Robust staffing:2- 3 staff or more depending on care needs of the residents -Has hired staff at night dedicated to caring for the residents -Specializes in memory care -Care Provider makes delicious meals and desserts focusing on organic ingredients and whole foods approach   SERVICES PROVIDED:

• 24 hour supervision
• Meals: nutritious home cooked meals 3  times a day and snacks.  Per client's preferences & dietary restrictions.
• Activities of Daily living: assistance with feeding, walking, transfers, bathing, toileting/incontinence care
• Medication Management
• Monitoring Vital Signs
• Exercise: Offered daily and as needed;  per doctor’s instructions
• Activities: Individualized per client's preferences
• Laundry
• Transportation: Caregiver may provide free transportation as part of their service to doctor’s appointments for client’s able to transfer in and out of car. For clients requiring more assistance, caregiver can arrange for private transport which generally is an out of pocket expense for families/clients.
