SERVICES PROVIDED:

• 24 hour supervision
• Meals: nutritious home cooked meals 3  times a day and snacks.  Per client’s preferences & dietary restrictions.
• Activities of Daily living: assistance with feeding, walking, transfers, bathing, toileting/incontinence care
• Medication Management
• Monitoring Vital Signs
• Exercise: Offered daily and as needed;  per doctor’s instructions
• Activities: Individualized per client’s preferences
• Laundry
• Transportation: Caregiver owns a medical transport van for medical appointments which is included in  the monthly rate. Escort provided by care provider.
