About This Home

This Adult Foster home is located in Salt Lake. Conveniently located near town and close to H-3. This home has been renovated,  home is airy and bright with high ceilings. The private room is expansive and measures 140 sq. ft.  Room is conveniently located directly across from the main bathroom. NOTE: Care Provider may add A/C upon request (additional fee may apply). SERVICES PROVIDED:
  • 24 hour supervision
  • Meals: nutritious home cooked meals 3  times a day and snacks.  Per client's preferences & dietary restrictions.
  • Activities of Daily living: assistance with feeding, walking, transfers, bathing, toileting/incontinence care
  • Medication Management
  • Monitoring Vital Signs
  • Exercise: Offered daily and as needed;  per doctor’s instructions
  • Activities: Individualized per client's preferences
  • Laundry
  • Transportation: Caregiver may provide free transportation as part of their service to doctor’s appointments for client’s able to transfer in and out of car. For clients requiring more assistance, caregiver can arrange for private transport which generally is an out of pocket expense for families/clients.
